Is that pressing the thumb against the same side of the pushrim as the palm, or a pinching motion?
If the former, I'm afraid I'm... kind of out of suggestions :-/ If you've got your thumb on the other side of the pushrim to your palm, however, (a) try applying pressure from one side with your palm only, and (b) it's possibly worth looking into whether you can get some hand physio. (I've just had a round of hand physio on the NHS at UCH, having been referred there by the hypermobility clinic; I don't understand hands well enough to know which of the exercises I've been doing might be appropriate for you, unfortunately -- I know that Yet More Interfacing With The NHS is rarely anybody's idea of a good time, but if it's possible for you it might be something to bear in mind :-/)
