[migraine] achievement of the day

... (other than not having a migraine any more, that is...) was writing myself up a pain scale for use with the UCLH-provided headache diary.
They want me to assign a "disability score from the headaches (0-10)" to each day. They do not provide definitions for these numbers. For my own purposes I am using a scale from 0-5, because on reflection it provided a level of granularity I wouldn't agonise over:
My Achievement was expanding this to an eleven-point scale so that I can actually have a stab at filling this damn form out for them.
Is this optimal? Almost certainly not! But it does include indicators of functional impairment and, crucially, it exists (and I've written it out on the piece of paper for ready reference), which means I might actually be able to fill the thing out. I need to decide how much of February I'm willing to try to retroactively translate, but definitely I can fill things in from the beginning of March, more-or-less.
(Today is the first day this week I have been below a 9. It's been nice.)

0: no headache
1: very mild headache - only noticeable if I pay attention/consciously check in
2: mild headache - I notice it spontaneously, but it's not significantly distracting
3: moderate headache - impairs concentration
4: semi-functional migraine
5: non-functional migraine
My Achievement was expanding this to an eleven-point scale so that I can actually have a stab at filling this damn form out for them.
0: no headache
1: very mild, intermittent; fades immediatealy out of awareness
2: very mild; takes a few minutes to fade out of awareness
3: mild, intermittent
4: mild, persistent; spontaneous awareness for >10 minutes multiple times throughout day
5: moderate; struggle to concentrate on writing complex text
6: moderate; struggle to concentrate on writing simple text
7: migraine features; can e.g. do simple food prep w/ sunglasses & supervision
8: migraine features; can tolerate sitting up & using laptop in low light w/ sunglasses
9: migraine features; horizontal, in dark room, w/ sunglasses & occasional screen use
10: migraine features; horizontal, in dark room, w/ eyemask, mostly dozing (& feeling sorry for myself)
