Things that occurred to me after pressing return: gel tyres, hybrid of pneumatic and solid, by filling a hollow tyre with gel. Hybridizes the advantages and disadvantages of both.

Back width can be a similar issue to arm-rests for pushing, I find the back of my NHS clunker is wide enough to interfere with my push motion, so I normally use it with the back folded.

Elevating leg rests. I have at least one friend who uses these regularly, and I've occasionally used them myself on things like airport chairs. These are an alternative to swing-away footrests, going on the same tube mounting, and allow you to keep one or both legs elevated if you can't bend a knee. Obvious disadvantages in having a leg stuck out in front for getting through tight spaces and opening doors. Available as an aftermarket add-on, but only if you have swing-away footrest to start with.
